What to Know

Sciatica refers to symptoms of pain and discomfort felt along the sciatic nerve’s pathway. Lumbar scoliosis in adults is the most closely associated with sciatica as the sciatica nerve starts in the lumbar spine. Not everyone with lumbar scoliosis experiences sciatic nerve pain, but it is a common complication.

Scoliosis causes the spine to curve unnaturally to the side and rotate, and particularly with nerve involvement, symptoms can be widespread. When scoliosis develops in the lumbar spine, sciatic nerve pain is a common effect as the spine’s unnatural curve and twist can compress the sciatic nerve.

The best way to minimize the potential effects of scoliosis, including sciatic nerve pain, is proactive treatment.

Primary Scoliosis Effects

The most important thing to understand about scoliosis effects is that their nature is to change over time; scoliosis is progressive (1).

Ranging widely in severity, the effects of mild scoliosis can be subtle, while severe scoliosis is associated with overt symptoms that can include nerve involvement (1).

Patients initially diagnosed with mild scoliosis need to understand that without treatment, symptoms are likely to increase alongside progression. Progression also makes scoliosis more complex to treat.

A goal of proactive nonsurgical treatment is to prevent progression, increasing scoliosis effects, and potential complications.

Primary symptoms of childhood scoliosis include postural changes such as uneven shoulders and hips, and the main symptom of adult scoliosis is pain; scoliosis pain can involve the back, the spine’s surrounding muscles, supportive structures, and radiating pain due to compressed nerves (1, 2).

Scoliosis pain is more closely associated with adult scoliosis largely due to compression and degeneration; scoliosis becomes compressive once skeletal maturity is reached, so nerve compression, in particular, is more likely in cases of adult scoliosis (2).

The Sciatic Nerve

The sciatic nerve is a mixed nerve with an extensive pathway starting in the lumbar spine and extending down the back of the lower body: pelvis/hip, buttock, leg, and foot (3).

As the thickest and largest nerve in the body, the sciatic nerve contains both sensory and motor fibers, and if it’s compressed, its health and function can be disrupted (3).

Symptoms of sciatic nerve irritation can involve sensory and muscle issues; the sciatic nerve provides sensation to the skin of the lower leg and foot and controls muscles in the knee and lower leg (3).

Compression is uneven and/or excessive pressure, and if the sciatic nerve is compressed, it can become irritated, inflamed, and/or impinged.

Sciatica

The term sciatica isn’t a clinical condition; instead, it references a set of symptoms experienced along the sciatic nerve’s pathway due to irritation (4).

Adults with degenerative lumbar scoliosis are the most likely to develop related sciatic nerve pain, particularly when severe and/or if left untreated.

One of the main roles of each spinal section is to protect the spinal cord so the 31 pairs of spinal nerves can function optimally, including the sciatic nerve in the lumbar spine.

If scoliosis has caused the lumbar spine to curve and twist unnaturally, the misalignment can cause sciatic nerve compression and irritation.

Common symptoms of sciatic nerve irritation include sharp shooting pain that radiates down the back of the lower body (4).

Sensations of tingling and/or numbness can also develop anywhere along the sciatic nerve’s extensive pathway, and muscle weakness in the affected leg and/or foot is also common (4).

Most sciatic nerve pain is felt down the body’s left side, but it can also affect the right side, and while less common, both sides can be equally affected (4).

Sciatica pain can vary in intensity, with some patients suffering from periodic flare ups during which time symptoms intensify and abate, while others suffer from chronic and debilitating sciatic nerve pain (4).

When nerve pain is chronic, it can disrupt mobility and quality of life, and if left untreated, sciatic nerve compression can lead to permanent sciatic nerve damage and pain.

Scoliosis and Sciatica

When sciatic pain is caused by scoliosis, the underlying cause of the pinched sciatic nerve has to be addressed: the uneven forces of the scoliosis.

Scoliosis-related sciatic-nerve compression can cause severe pain, and for any type of sustainable long-term pain relief, the misaligned spine needs to be addressed and balance restored (5).

Improving the spine’s balance and stability through a curvature reduction and improved spinal support can help take pressure off the spinal canal and the sciatic nerve (5).

A common complication of adult lumbar scoliosis is sciatica, and sustainable pain management requires scoliosis treatment that improves the abnormal curvature of the spine and its related effects.

Degenerative scoliosis affects older adults and is almost always progressive and painful; degenerative scoliosis largely affects the lumbar spine and involves disc degeneration that can also contribute to sciatic nerve compression (4, 5).

Remember, because scoliosis is progressive, even patients diagnosed with mild scoliosis can progress to become moderate or severe, and the more severe a scoliosis becomes, the more likely nerve involvement and related complications are (1, 2, 5).

So for those recently diagnosed with lumbar scoliosis and who aren’t experiencing sciatic nerve pain, that doesn’t mean nerve pain can’t become a future issue.

Sciatica Scoliosis Treatment Options

Patients experiencing sciatica symptoms are most likely to involve adults with lumbar scoliosis, and in order for sciatic nerve health and function to be restored, scoliosis, as the underlying cause, needs to be addressed (1, 2, 5).

Because scoliosis is structural, treatment has to address the structural nature of scoliosis for a lasting impact on its effects, and nonsurgical treatment focuses on improving spinal alignment, balance, and stability, while also working towards improving the strength and balance of the spine’s surrounding muscles for less pressure on the spine, its individual structures, and nerves (1).

ScoliCare Clinics around the world have been improving the lives of people with scoliosis for 15 years; the ScoliCare approach is proactive, nonsurgical, and involves customized treatment plans combining scoliosis-specific rehabilitative exercise (ScoliBalance®) and corrective bracing (ScoliBrace®) (6, 7, 8).

Scoliosis-specific Chiropractic BioPhysics® care can be used as an adjunctive treatment that includes manual adjustments focused on improving the spine’s position and alignment (9).

ScoliBalance® combines the best of what scoliosis-specific chiropractic care and established and proven scoliosis-specific exercise-based treatment programs (the Schroth Method and the Scientific Exercise Approach to Scoliosis) have to offer in terms of corrective potential (7, 8, 9).

Scoliosis-specific rehabilitative exercise can work towards improving the spine’s flexibility for pain relief and more responsiveness to treatment, while also improving the spine’s surrounding muscle strength and balance for more spinal support (7, 8, 9).

When combined with the corrective ScoliBrace®, the scope of nonsurgical treatment widens as the ScoliBrace® designed to work with movement so complements ScoliBalance® and helps support the spine’s healthier position and the body’s upright posture (6, 7, 8)

So in cases of lumbar scoliosis that involve sciatica, the sciatic nerve pain will be reduced as improvements to the scoliosis are made.

Conclusion

Scoliosis can cause sciatic nerve pain as a related complication of severe and/or untreated lumbar scoliosis, and because scoliosis becomes compressive when growth stops, nerve compression is more closely associated with adult scoliosis.

Because scoliosis is progressive, its effects are likely to increase over time, so it’s important for patients recently diagnosed to understand that symptoms experienced at the onset of scoliosis don’t reflect how the condition can change over time.

The longer scoliosis is left untreated, the more likely it is to continue progressing, and the more potential there is for developing related complications such as sciatic nerve pain.

When the lumbar spine is exposed to uneven pressure, it can affect the sciatic nerve that starts in the lower back through compression, irritation, inflammation, and/or impingement.

Scoliosis-related sciatica pain can range from mild to severe, and while medications can help provide short-term pain relief, for sustainable long-term sciatica pain solutions, the scoliosis needs to be treated proactively to prevent increasing pain and restore sciatic nerve health and function.

References:

Janicki JA, Alman B.
