Treatment for a pars defect usually begins with reducing painful activities, avoiding repeated back extension, physiotherapy to improve spinal support, and sometimes bracing. Surgery is considered only in selected cases where symptoms persist or instability is more severe.

A pars defect, also called spondylolysis, is a small stress fracture in part of the vertebra called the pars interarticularis. Symptomatic spondylolysis can involve lower back pain and radiating pain in the buttocks and thighs. Treatment options include conservative treatment or surgical treatment.

Pars defect can vary in severity, but if left untreated, it can cause increasing pain, instability, degeneration, and spondylolisthesis.

Pars Interarticularis

The spine is the body’s central structural anchor, and its overall strength and function depends on the health of its individual structures, including the pars interarticularis.

Vertebral bodies (bones of the spine) are separated by intervertebral discs, and the pars interarticularis is a small thin bridge of bone located on the back of each vertebra connecting the facet joints of each vertebral level (1).

The facet joints connect the vertebrae, give the spine structure, and facilitate a healthy degree of movement.

The pars interarticularis is an essential stabilizer that facilitates the spine’s ability to bend and twist for a wide range of motion, but we also don’t want too much movement within the spine, and the pars interarticularis is generally considered the weakest part of a vertebral body so is susceptible to overuse and stress fractures (1, 2).

If the pars interarticularis is stressed and weakens, it can cause pain and disability.

Pars Defect and Pain

Repetitive stress can strain the thin structure of the pars interarticularis, weakening it over time. Repetitive high-impact activities such as gymnastics, diving, certain dance styles, and heavy weight lifting can contribute to a pars fracture (2).

Hyperextension of the spine can strain the spinal structures, including the pars.

The pars provides the spine with structural stability by linking the facet joints and stabilizing each vertebral level, so a break can cause excessive movement, increasing instability, and lower back pain (2).

A pars defect is most common in the lower back, and this is due to the strain placed on the lumbar vertebrae (2); they are the largest and strongest vertebrae because they have to support the weight of the spinal sections above, the torso, and feel the strain of repeated strenuous movement such as lifts, bends, and twists (3).

Because of the many roles of the lumbar spine, most people will experience lower back pain at some point in their lives, and a pars defect that’s unaddressed can also contribute to the start of degenerative changes to the spine’s health and function (4). If the bony connection between certain vertebrae is broken, spinal discs have to absorb more stress and the early onset of disc degeneration can develop (4, 5).

Recognizing the warning signs of a pars defect may help prevent the development of chronic pain, changes to disc health, and may help a pars defect heal without surgery.

Pars Defect Warning Signs

Adolescent athletes are the most vulnerable to a pars defect, and persistent pain in the lower back that worsens with activity and improves with rest is a primary symptom (2).

Pars-defect pain can increase with certain movements such as backward bends and rotations (2).

The onset of pars defect symptoms isn’t always clear; they can develop slowly over time or suddenly with acute back pain and deep muscle pain (2).

An additional, often subtle, warning sign involves tight hamstrings that make it difficult to maintain a wide healthy stride (2).

Some people may also feel pain in the buttocks or thighs, especially if there is irritation around the lower spine or associated slippage (2).

Deep muscle pain and muscle spasms can develop in the lower back that may be sensitive to touch and pressure changes (2).

If changes in position ease discomfort, this can be due to creating more space within the spinal canal for pressure relief.

Knowing the early signs of ongoing pars stress can lead to early treatment, the prevention of further injury, and help adolescents return to much-loved sports sooner. In addition to the physical-health benefits of regular exercise, the mental-health benefits of dedicated exercise/team sports are well known. The sooner young athletes are assessed and treated, the sooner the healing process can begin.

If any of the aforementioned warning signs are noted, professional assessment is needed so an untreated pars defect doesn’t cause spondylolisthesis.

Spondylolisthesis

Spondylolisthesis develops if a lack of stability causes one vertebra to slip out of its natural position, and growth spurts increase the risk of spondylolysis and spondylolisthesis (6). Spondylolysis diagnosed early in children is important because it means the condition can be managed proactively during growth spurts (6).

When a fractured pars separates and there is too much instability and movement, the injured vertebra can slip forward onto the vertebra below (6).

A slipped vertebra is assessed as low grade or high grade with high-grade slippage involving more than 50 percent of the fractured vertebra slipping onto the vertebral body below (6).

Patients who have experienced high-grade slippage are likely to experience chronic lower back pain and radiating pain due to nerve compression (pinched nerve) (6).

More severe slippage may require specialist review, and in some cases, surgery may be discussed if pain, instability, or nerve symptoms persist (6).

Treatment for Pars Defect

Determining the underlying cause of the fracture is key, and X-ray imaging tests are needed to see what’s happening in and around the affected vertebrae. Magnetic resonance imaging (MRI) tests can be also used to look for areas of bone swelling that indicate the early stages of a stress fracture (7).

MRIs can assess soft tissue damage, disc damage, and nerve compression, and it can also be used to monitor the bone healing process (7).

Because most pars defect cases involve overuse and repetitive stress from a specific injury, rest from certain sports and/or long-term activity modification is the first line of treatment (8).

A break from activities that involve hyperextending the spine is needed to support the healing process, and nonsteroidal anti-inflammatory medications reduce inflammation and relieve pain (8).

Physical therapy that focuses on strengthening abdominal and back muscles can increase the spine’s support and stability; a spine that’s surrounded by strong and balanced muscles has less pressure on its individual structures, like the spinal discs, facet joints and pars interarticularis (8).

Specialized exercise and stretches can also work towards loosening tight hamstrings to reduce their pull on the pelvis (8).

A rigid lower-back brace may also be recommended to limit movement and to help the pars interarticularis fractures reconnect and heal (8).

In severe cases and when nonsurgical treatment options have failed, surgery may be recommended and can include a direct repair of the pars defect or lumbar spinal fusion surgery (9). A direct repair can involve attaching the two halves of the fracture with long metallic screws to increase stability (9).

Conclusion

If lower back pain is persisting, worsening with sport, or linked to repeated back extension, an assessment can help identify whether a pars defect is involved and what treatment options may be appropriate.

A pars defect can be addressed with rest and activity modification, physical therapy, a supportive lumbar brace, and in severe cases where spondylolisthesis is causing increasing instability, persistent pain and nerve compression, surgical intervention may be recommended.

For parents of adolescent athletes, recognizing the warning signs of a pars defect allows for more time to understand the injury, its contributing factors, and the next best steps to take. A personalized early treatment plan may also help young athletes return to regular daily activities and sports faster.

References:

Mansfield JT, Wroten M.
